The Diabetes Self-Management Program provides comprehensive instruction on the following topics:

     * Type 1 & Type 2 Diabetes
      * Home Blood Sugar Management
      * Diabetes Management
      * Meal Planning
      * Label Reading
      * Medications
 
We are committed to providing quality education based on the American Diabetes Association guidelines for diabetes. Our staff consists of Certified Diabetes Educators, Registered Nurses and Registered Dietitians.

Program Services include: Individual Counseling and Education, Diabetic Assessment, Medical Nutrition Therapy, Insulin Preparation and Injection, Self Monitoring Blood Glucose and follow-up inquiries.

Group/Class Education:

       * 9 Hour series of classes for Diabetes
          Self-Management
       * Blood Glucose Meter Class
       * Insulin Preparation and Injection Class
       * Gestational Diabetes Class


The Great Outcomes program services both Dallas and Ft. Worth area locations. The out of pocket cost to you depends on the services ordered by your physician and the amount of your insurance reimbursement.
